Plane Healthy 2009 Minimum Participation Requirements:
To receive the $600 Plane Healthy Wellness Credit for 2010, the following three Plane Healthy requirements must be completed no later than October 31, 2009:
Complete a confidential Health Risk Assessment (HRA) online - Click Here
Complete a Plane Healthy Wellness Credit Form to document the completion of your physical or basic
wellness screenings. Basic wellness screenings will be offered onsite at Cessna in 2009, or you may go to your own personal licensed healthcare provider for a physical. Return form in reply envelope to:

New in 2009! Biometric screenings for cholesterol and glucose will be offered during some of the onsite wellness screenings. A finger stick to obtain a blood sample will be required for these screenings. Please note that the biometric screenings are optional and not required for the 2010 Wellness Credit.
Register and participate in at least two of the six offered behavior change programs and report adherence to guidelines online at www.cessnaplanehealthy.com.
