The following process will be required to appeal all decisions regarding completion status for the Plane Healthy Wellness Program.
FIRST LEVEL APPEAL
The first level appeal must be submitted in writing no later than January 15th of the corresponding program year. It must include a statement of why the program requirements were not met, include copies of any supporting documentation, and provide any other relevant information pertinent to making a determination. The appeal must be submitted to wellness@cessna.textron.com for consideration. If you do not complete your appeals request by January 15th as described above, the appeal will automatically be denied unless you provide documentation of extenuating circumstances as to why you were unable to meet the January 15th deadline. These circumstances will be reviewed and a waiver of the January 15th deadline may be considered.
SECOND LEVEL APPEAL
If your appeal is denied and you wish to request further consideration of your circumstances, you may submit a second level appeal in writing within 10 days of the date of denial to wellness@cessna.textron.com. The appeal must include the same information as described above plus a copy of the denial you received and any other information you feel is pertinent to your situation. The second level appeal will be considered by an individual who did not review your first level appeal. The decision on a second level appeal is final.
APPEALING A BEHAVIOR CHANGE PROGRAM
To appeal any behavior change program, you must submit the appeal in writing no later than 60 days after the conclusion of the program in order for the appeal to be considered. It must include a statement of why the program requirements were not met, include copies of any supporting documentation, and provide any other relevant information pertinent to making a determination. The appeal must be submitted to wellness@cessna.textron.com for consideration. If you do not complete your appeals request by the deadline as described above, the appeal will automatically be denied unless you provide documentation of extenuating circumstances as to why you were unable to meet the 60 day deadline. These circumstances will be reviewed and a waiver of the deadline may be considered. A second level appeal may be considered.
PLEASE NOTE: It is your responsibility to maintain a copy of all documentation you submit as a part of this appeals process, including dates of submission.
Participation
- Is participation in Plane Healthy voluntary? Yes.
- What happens if I don’t participate? You won’t receive the wellness credit for the corresponding year.
- Can my family members participate in Plane Healthy and receive a wellness credit as well? Yes. Spouses and Domestic Partners will be encouraged to participate by having an annual physical exam and completing the online Health Risk Assessment. Spouses and Domestic Partners will also have the opportunity to participate in the Behavior Change Programs, though they will not be required for credit. Dependents will not be eligible for participation at this time.
- I have a chronic health condition. Can I still participate in Plane Healthy? Yes. The purpose of Plane Healthy is to encourage and reward healthy behaviors and even though someone has a chronic health condition does not mean they cannot practice healthy behaviors.
Wellness Credit
- Who is eligible to receive a wellness credit? All Salaried Exempt and Salaried Non-Exempt employees who complete the minimum requirements of one of the three levels of commitment (tiers) for the corresponding Plane Healthy program year.
- I plan to waive Textron medical insurance. Can I still get the wellness credit? Yes, if you are a Salaried Exempt or Salaried Non-Exempt employee who completes the minimum requirements of one of the three levels of commitment (tiers) for the corresponding Plane Healthy program year.
- How do I know if I am qualified to receive the wellness credit? You can check the Plane Healthy website at www.cessnaplanehealthy.com to see your status. Accessing this link can be done from any computer with internet access.
- If I go above & beyond the minimum requirements, do I get extra wellness credit? There will not be any additional monetary credit, but you will receive the added personal benefits from leading a healthier lifestyle.
- How is the wellness credit paid? The wellness credit is a cash benefit prorated over each pay period throughout the year.
2010 Plane Healthy
- 1. Why did Cessna implement a tiered incentive plan? Plane Healthy strives to make
wellness a personal commitment. A tiered incentive program better allows employees to
choose their level of commitment and rewards individuals who may already be actively
participating in healthy behaviors.
- Why does only Tier 1 ($150) allow a screening in place of a physical? While
screenings can provide valuable insight into your health, a screening should not be a substitute for an annual preventive physical exam. To reflect the importance of the physical as part of a healthy routine, we have chosen to highlight this requirement in the top two tiers of the program.
3. Will screenings be provided onsite again this year? Biometric screenings, which
include a finger stick for blood sugar and cholesterol, will provided onsite in Wichita in a
very limited capacity once a quarter. Screenings will not be provided onsite at the outlying
facilities this year.
- Why does Tier 3 require participation in the seminar entitled “Understanding Your
HRA? Cessna recognizes not only the importance of providing employees the tools to improve their health, but also the necessity to explain how those tools work. Therefore, this seminar will provide an overview of how Cessna employees are doing in the realm of wellness and provide information on how this valuable tool can be utilized to help manage your care.
- If I work at an outlying Service Center, Columbus or Independence how will I
participate in the seminar for the Tier 3 credit? One of the Wichita seminars will be videotaped so that employees at outlying facilities can watch it at a later date. The seminar will be presented onsite in Independence later in the year. More details will be provided at a later time.
- Why am I required to specifically discuss tobacco use and my weight status with
my healthcare provider during my annual physical exam? According to the National Center for Health Statistics, an estimated 66% of U.S. adults are overweight or obese. One of the national health objectives for 2010 is to reduce the prevalence of obesity among adults to less than 15 percent. Overweight and obese adults are at increased risk for morbidity from hypertension, type 2 diabetes, coronary heart disease, stroke, gallbladder disease, osteoarthritis, sleep apnea, respiratory problems and some types of cancer. The Surgeon General has called tobacco use, including cigarette smoking, cigar smoking, and smokeless tobacco, “the leading preventable cause of disease and deaths in the United States.” An estimated 438,000 Americans die each year from tobacco related diseases, such as lung cancer, heart disease and chronic lung diseases like emphysema, bronchitis and chronic airway obstruction.
- Why do I have to get a dental exam in order to qualify for the Tier 3 credit? The Surgeon General reports 120 diseases can be diagnosed through the mouth including nutritional deficiencies, immune system problems, infection and other diseases and conditions. Scientific literature increasingly makes connections between gum disease and chronic diseases like diabetes, cardiovascular disease as well as pre-term births. That is why the American Dental Association also recommends a yearly check-up for denture wearers. It is for this reason we chose only a yearly visit to the dentist and did not require a visit every 6 months. If your dentist does not recommend a yearly exam, you may provide a note from the dentist stating such and you will still be eligible for the credit.
- Why is Cessna requiring my spouse or domestic partner to participate in a physical exam and the online Health Risk Assessment (HRA) as a part of the Tier 3 requirement? One of the most common requests we get with Plane Healthy is the inclusion of spouses/domestic partners. Therefore, we have added spouses/domestic partners to the program.
- Does my spouse or domestic partner have to participate if they are not a Cessna employee and are not covered by Cessna insurance? Your spouse/domestic partner must complete a physical exam and the online Health Risk Assessment in order for you to qualify for the Tier 3 level. The requirements of Plane Healthy are independent of insurance coverage.
- Can I still participate in Tier 3 if I don’t have a spouse or domestic partner? Yes. All employees who are interested in Tier 3 will be required to answer one of three questions on the Plane Healthy website regarding spouse/domestic partner participation. If you are single, you will simply answer as such.
- Can my spouse or domestic partner access the Plane Healthy website from home? Yes. The Plane Healthy website is an external link and may be accessed directly from home at www.cessnaplanehealthy.com.
- What if my spouse or domestic partner wants to participate but we do not have internet access at home? As an employee, you may request the required physical form for your spouse to have completed if necessary. Your spouse can call the wellness hotline at 1-888-FIT2FLY or 316-517-4000, to request a paper version of the Health Risk Assessment. You may also visit your local library for free internet access in many locations.
- What if I get married during the program year? Does my spouse/domestic partner still have to complete the requirements for the Tier 3 level? If you get married during the program year prior to September 30th your spouse/domestic partner will be required to complete the requirements of Tier 3 in order for you to get that level of credit. Any union that occurs after September 30th will not be subject to completing the spouse/domestic partner requirements for the current program year.
- Will the Plane Healthy Wellness Credit Form be mailed to my home for my healthcare provider to sign? Thanks to employee feedback, the Plane Healthy Wellness Credit Form, Dental Form and Spouse/Domestic Partner physical forms will be available by request through the Plane Healthy website. This allows employees to request the forms they need prior to their exams. The forms will be mailed to your home once requested through the website. Please allow two weeks for delivery and receipt of the form.
- Where can I find a summary of the requirements for each tier level? If you visit www.cessnaplanehealthy.com and click on the tab for 2010 Plane Healthy at the top of the home page, you will find each tier level broken down along with the links to the necessary forms, HRA, and seminar registration information where applicable.
- Plane Healthy is requiring participation in more behavior change programs this year. What if my schedule is too busy to participate in the required number of programs? Cessna understands that employees lead busy adult lives, both at work and home. Fortunately, the behavior change programs are designed to be incorporated into daily activity without the need for “scheduling” time for wellness. If you feel that you are unable to meet the requirements for the maximum incentive, you may also participate at a lower tier level which requires less commitment.
- How will I know if I have met all of the requirements for a specific tier? You can check your status online at anytime at www.cessnaplanehealthy.com. Click on the link for PH Status on the home page and enter your PH Code. There, the requirements of the program will be broken down for you and each requirement will say “complete” or “incomplete” as appropriate. Once you have earned the credit for the appropriate tier level, your status will change from “credit not earned” to “credit earned.”
- Why did the wellness requirements and incentives change this year? The purpose of any wellness program is to continuously challenge and reward individuals to actively take steps to improve their health and wellbeing. The changes in this year’s program reflect that opportunity and allow individuals to choose their own level of commitment to wellness.
Commonly Asked Questions
- If I completed the Health Risk Assessment (HRA) last year, will I be asked to complete it again this year? Yes. You will be required to complete the HRA each year between November 1st and October 31st of the corresponding program year. All three tier levels require completion of the HRA. This requirement remains consistent as it is a tool that is used for employees to monitor their year-over-year progress and to give them a starting point for a conversation with their healthcare provider about their health.
- Will my responses to the HRA be confidential? Yes, Cessna is partnering with a third-party vendor to provide and process the HRA responses. They will only provide a report to Cessna in aggregate (group) form. No individualized data will be provided to Cessna.
- Do I need Internet access to participate in the HRA? You can access the Plane Healthy website at work without internet access, or you can login from home using an internet provider. If you do not have regular computer access, contact either your supervisor or HRBP who will help you log onto a provided computer at work.
- What is a behavior change program? Behavior change programs are fun, easy programs aimed to promote healthy behaviors. The programs will be 4-7 weeks in length with varying degrees of time commitment. They will be offered six times per year and will consist of an initial mailer to your home promoting the program, online registration and keeping an online record of compliance.
- There is no mention of regular exercise as a minimum requirement. Is this accurate? Exercise is not a requirement for Plane Healthy; however, we highly encourage this healthy behavior.
- Is someone going to track that I’ve met the requirements? Yes. Integrated Health & Wellness, a third party supplier contracted with Cessna, will manage this aspect of Plane Healthy in order to maintain confidentiality.
- How will it be tracked? A Plane Healthy Wellness Credit Form, dental form and spouse/domestic partner form, will be used to confirm that you received either a physical or biometric screening. The form(s) will be mailed to you for completion. It will require a signature from a licensed healthcare provider in order to qualify. Once complete, you must submit the form to Integrated Health and Wellness with a postmark no later than October 31st of the corresponding program year. To track completion of the HRA, the third-party supplier will provide Cessna with a list of employees who completed the HRA. You will be identified by the supplier by the unique code that will be provided on your HRA form. Cessna will only see names of employees who completed the HRA. Individual responses will NOT be seen by Cessna.
- Can my physician submit their own physical form to receive the credit? No. You will be required to use the provided Plane Healthy Wellness Credit Form with your unique identifier code.
- When is the latest I can complete a physical and/or biometric wellness screenings in 2010 and still complete the 2011 Plane Healthy requirements? Your Wellness Credit Requirement Form, dental form and spouse/domestic partner form must be postmarked no later than October 31, 2010; therefore, your physical and biometric wellness screenings must be completed in time for this to occur.
- If I miss the postmark deadline for the form(s), will my form still be accepted if my physical was completed prior to the October 31st deadline? No. The postmark deadlines as well as the deadlines for completion of the other requirements will be strictly enforced.
- I am a new salaried exempt/salaried non-exempt employee. Do I qualify for the wellness credit? New salaried exempt/salaried non-exempt employees who begin their job prior to September 30th of each year will be eligible to participate for the credit in the corresponding year. Those hired on October 1st or after will be eligible for the credit the following year.
Physicals and Basic Wellness Screenings
- If I just had a physical or wellness screening in 2009 why do I have to have another one in 2010? Prevention and early disease detection are crucial for maintaining good health. Therefore, it is important to establish a relationship with your healthcare provider and monitor your personal risk factors on a yearly basis.
- Will my pre-employment, DOT, FAA or annual work-related physical count toward the physical requirement for the program? No. These physicals are task specific and are not considered a comprehensive preventive exam. Therefore, they do not count toward the requirements of Plane Healthy.
- Will my Executive Physical through Cessna qualify for the minimum requirements? Yes. The Executive Physical is a comprehensive physical exam and will count toward the minimum requirement for the physical.
- Will a “well woman” exam qualify for the minimum requirements? While we encourage and prefer for you to have a more comprehensive physical exam, we recognize that some physicians provide more thorough well woman screenings than others. Therefore, it will be between you and your physician to determine if the physical exam you have is comprehensive enough to meet the minimum qualifications.
- Will all of the wellness screenings and events be offered during normal working hours? The times that programs are offered will vary. It is expected that participation will require some personal responsibility and activities will need to be attended outside of your normal working hours.
- I want to get an annual physical, but I do not have a primary care physician. How do I get one? We encourage you to contact your medical insurance company to obtain a list of in-network providers. If you have United HealthCare, you can retrieve the information from www.myuhc.com.
- I have United HealthCare. Isn’t my preventive care visit paid 100%? Yes. You can check the Summary Plan Description at www.fidelity.netbenefits.com which outlines this benefit. It might be a good idea to remind your healthcare provider to list “preventative care” as the reason for the visit to ensure the service is coded appropriately and United HealthCare can process the claim accordingly.
- If I don’t have United HealthCare how will my physical be paid? It will be paid according to the insurance plan design of your healthcare carrier.
- If I choose to participate in the biometric screenings that are offered onsite at Cessna, will I have to pay for them or will my insurance be billed for these services? No, you will not be charged any fees and your insurance will not be billed for the screenings, if you complete them onsite at Cessna. This service is being provided to you as a part of the Plane Healthy programming.
Plane Healthy Wellness Credit Form
- What do I do if I lose my Plane Healthy Wellness Credit Form, dental form or spouse/domestic partner form? Additional forms may be requested through the Plane Healthy website.
- Why are there no electronic versions of the Plane Healthy Wellness Credit Form, dental form or spouse/domestic partner forms? Scantron forms are being utilized in order to provide a simplified form for your healthcare provider to sign and an efficient method for processing the forms.
- If I lose my form(s) can I make a copy of someone else’s blank form? No. Photocopies and Faxes will not be accepted. THIS POLICY WILL BE STRICTLY ENFORCED.
- Should I keep a copy of my form? Yes. You should keep a copy of your signed form for your records.
Website Access
- Why does my status on the Plane Healthy website show incomplete if I have already completed the HRA? If you completed the HRA within the last week, it might not be reflected as the completion information is downloaded to the website on a weekly basis. If it has been longer than 10 days since you completed your HRA, please contact wellness@cessna.textron.com so that the issue can be investigated.
- I just started at Cessna and I can’t log-in to the Plane Healthy website. When will my information be available? New hires and transfers will be downloaded into the system once a month. Even if you have not been added to the system yet you can still access the wealth of information on the website and work towards your minimum requirements for the 2009 wellness credit.
- I have forgotten my password for the HRA site. How can I log-in? If you forgot your password, simply click on the link on the log-in page for “Forgot Password” and your password will be emailed to the email address you provided when you registered.
Miscellaneous
- I hear Textron’s looking at a wellness program. Will that change ours? Textron is in the very early stages of encouraging and supporting wellness efforts at the business units. At this time, we don’t know what the impact may be.
- Will there be regular communication about Plane Healthy? Yes. We will use regular communication vehicles as well as the Plane Healthy website, which will be linked from ERIC.
- What if my question is not answered in the FAQ’s? Send additional questions to the Plane Healthy Outlook mailbox at wellness@cessna.textron.com or call the Plane Healthy hotline at 316-517-4000.
